Upon listing an additional article on this list, the Director of Maintenance or delegate, will forward a copy to the FAA within one week by Fax or by Email or hard copy sent to the following address: FAA 6961 FLIGHT ROAD RIVERSIDE, CA 92504 Prior to a change the Director of Maintenance will conduct a self-audit in accordance with FAR 145.209. Items considered in the self-audit will include but not be limited to housing, facilities, equipment, material, technical data, processes, and qualified technicians. Form # CAS087 shall be used for the self-audit, instructions are detailed in QSM-OP-102. Seft-audit form # CAS087 will be kept on file for 12 months. CERTIFIED AVIATION SERVICES LLC.
