SHIPPER’S CERTIFICATION FOR LIVE ANIMALS
(to be completed in duplicate)
This is to certify that;
1.  
The shipper acknowledges the following possibilities associated with transporting animal(s). The environment of air transportation includes changes in temperature, humidity, air pressure fluctuation, and exposure to light and noise, which may cause stress or have negative effects on animal(s), especially, small dogs, snub nose dogs and aged dogs. Possible effects on the animal(s) include situations such as physical changes, weakness or even death of the animal(s) may occur.

2.  
In addition to having completed advance arrangements, this consignment is properly described and packed, and is in proper condition for carriage by air according to the current Edition of the IATA Live Animals Regulations and all applicable carrier and governmental regulations.

3.  
The animal(s) of this consignment is (are) in good health and condition. Animals taken from the wild have been appropriately acclimatized.

4.  
This consignment does not include animals prohibited by governments, such as described in the Convention on International Trade in Endangered Species of Wild Fauna and Flora. Animals contained in this consignment can be legally carried and imported into the country of ultimate destination and through the transit countries en route. Applicable permits/certificates are attached to the air waybill.

5.  
No mammals are tendered for transportation having given birth in the last 48hours before the start of the journey. When applicable, official veterinary certificate documents are attached to the air waybill for pregnant mammals.

6.  
The animal(s) is not of the Bulldog or the French bulldog breed. The shipper accepts that the carrier does not accept any Bulldog or French bulldog breeds for carriage.

7.  
The shipper accepts that carriers will not be liable for any loss, damage or expense arising from death due to natural causes, or death or injury of any animal caused by the conduct or acts of the live animal itself or of other animals, such as biting, kicking, goring or smothering, not for that caused or contributed to by the conditions, nature or propensities of animals. In no event will carrier be liable for death or injury to an animal attendant caused or contributed to by the condition, conduct or acts of animals.

8.  
The consignee has been advised of the flight details in order to arrange immediate collection on arrival. The shipper is liable for all charges and expenses resulting from the consignee’s refusal or failure to take delivery of this consignment, including the charges for returning the consignment to the airport of origin.
